ADMISSIONS PROCESS

"The admission process for Change of Life Group Home begins with an initial assessment conducted by our intake team. During this assessment, we gather information about the individual's medical history, current mental health needs, and personal goals.

Following the assessment, a meeting is scheduled with the individual and their family members or support system to discuss the services and support available at our facility. This is an opportunity for the individual and their loved ones to ask questions, express concerns, and determine if Change of Life Group Home is the right fit for their needs.

If the individual decides to move forward with admission, our team works collaboratively with the individual, their healthcare provider, and any other relevant professionals to develop a personalized treatment plan. This plan outlines specific goals, strategies, and interventions to support the individual's mental health and overall well-being during their stay at our facility.

Once the treatment plan is in place and all necessary paperwork is completed, the individual is welcomed into our community-style living environment. Our staff provides ongoing support, guidance, and encouragement to help residents adjust to their new surroundings, build relationships with fellow residents, and actively participate in the services and activities offered at Change of Life Group Home.

Throughout their stay, our team regularly reviews and reassesses the individual's progress to ensure that their needs are being met and that they are on track towards achieving their personal goals. We strive to create a positive and empowering experience for each resident, helping them navigate their journey towards recovery and a fulfilling life."
